When you use the appointment request form on this website, we collect the information you type into it: your name, date of birth, gender, phone number, email address, preferred language, mailing address, employer name (if any), self-employment type (if any), estimated annual income, and whether you consent to receive text messages. We do not collect a Social Security number through this website, and we do not accept identification, immigration or tax documents through this website — bring those to your appointment, or ask your agent for a secure link.

Why we collect it

We use this information to contact you about your request and to help you with Marketplace, dental or vision enrollment. We do not use it for any other purpose.

Where your information goes

Information you submit through this website is stored in our client-management system, where a licensed SimpleCare Insurance agent can review it and follow up with you.

How long we keep it

We keep client records for as long as insurance record-keeping requires, up to 10 years after your last policy activity.

We do not sell your information

We do not sell your personal information, and we do not share it with third parties for marketing or promotional purposes.

Correcting or deleting your information

To review, correct or request deletion of the information we hold about you, email info@yoursimplecare.com.

Cookies

This website does not set cookies. The Cloudflare Web Analytics beacon that helps us understand site traffic does not use cookies or track you across other websites.

Children

This website is not directed to children under 13, and we do not knowingly collect information from them.
