Tell us how we can help
This form sends a licensed SimpleCare Insurance agent the basics needed to call you back about Marketplace, dental or vision coverage. It never asks for a Social Security number, and identification, immigration and tax documents are never collected through the website — bring them to your appointment or ask your agent for a secure link. There is no charge for the call or the appointment.

How we use your information
We use the information you provide to respond to your request and help with your insurance options. We do not sell your personal information. Do not send sensitive documents through this form.
Your information
Only the basics an agent needs to call you back. This form never asks for a Social Security number, and no documents are uploaded here.
What happens next
A licensed SimpleCare agent reads what you sent and calls you at the number you gave. There is no charge for the call, and nothing is sent to the Marketplace until you have seen your options and agreed.

Nothing is uploaded here. Have these ready for the appointment, and your agent confirms which of them apply to your household.
- A photo identification for each adult on the application
- Proof of your current address
- Your best estimate of household income for the year
- Your immigration document, when it applies
- Your current policy information, if you already have coverage
Can I get health insurance if I am not a U.S. citizen?
To enroll through the Marketplace you generally need to be lawfully present in the United States and not have access to other qualifying coverage; many lawfully present immigrants qualify for coverage and for financial help. Bring your immigration document and an estimate of your yearly income and we will check your eligibility together, free.
How much does health insurance cost in Miami?
There is no single price. Your monthly premium depends on your household size, expected annual income, age, zip code and the plans available where you live, and you may qualify for financial help that lowers it. We calculate your real monthly number and out-of-pocket costs with you in the free appointment, before you choose.
Do you charge for your help?
No. SimpleCare Insurance is paid by the insurance carrier when you enroll, so the appointment, the plan comparison, the application and your renewals cost you nothing. Simple Tax charges separately for tax preparation, and you are told that price before any work begins.
When can I enroll in a health plan?
Open Enrollment for the coming plan year runs Nov 1 – Dec 15. Outside that window you can enroll if a qualifying life event opens a Special Enrollment Period, for example a move, marriage, new baby or loss of other coverage. Dates can differ in states that run their own Exchange. We can check your situation, free.
My income changes every month. What income do I report?
Report your best reasonable estimate of the household income you expect for the whole year — for self-employment, income after business expenses. If it turns out different, tell us and we update the Marketplace application. Keeping it current reduces surprises when the premium tax credit is reconciled on your federal tax return.
Can I keep my doctor?
Often, but it must be checked plan by plan. Every plan has its own provider network and drug list, so bring the names of your doctors and your current medicines to the appointment. We look each one up in the plans available to you before you enroll, rather than after.
Would you rather book directly?
If you would rather choose the time yourself, pick your agent and reserve a free appointment, in person in Miami or by phone. You do not need to send this form first.
