What to bring to your appointment

Bring a photo ID, proof of your address, an estimate of your household income for the year and, when it applies, your immigration document. SimpleCare Insurance does not accept identification, immigration or tax documents by ordinary email or through this website: bring them to your appointment in Miami, or ask your agent for a secure link.

Identification

Bring a photo identification for each adult on the application, and something that shows your current address.

  • A state-issued driver license or identification card
  • A U.S. passport
  • A Permanent Resident Card or an Employment Authorization Document
  • A recent utility bill, lease or bank statement showing your address

Proof of income

The Marketplace sets financial help from your best estimate of household income for the year. Bring whatever shows that estimate most clearly.

  • Recent pay stubs
  • Your most recent federal return
  • Form 1099 for contract or self-employment work
  • A profit and loss summary if you work for yourself
  • Award letters for Social Security, unemployment or pension benefits

Immigration and citizenship documents

To enroll through the Marketplace you generally need to be lawfully present in the United States and not have access to other qualifying coverage; many lawfully present immigrants qualify for coverage and for financial help. Bring your immigration document and an estimate of your yearly income and we will check your eligibility together, free.

For your taxes

Simple Tax prepares returns alongside the agency. Bring these to a tax appointment.

  • Photo identification, and a Social Security card or ITIN letter for everyone on the return
  • Form W-2 from each employer
  • Form 1099 for contract, gig or self-employment work
  • Form 1095-A if anyone had a Marketplace plan
  • A record of business expenses if you file a Schedule C
  • The prior return, if we did not prepare it

Bring your documents to your appointment in Miami, or ask your agent to send you a secure link. There is no upload on this website.

Never email a photo of your identification, your immigration document or your tax forms. Ordinary email is not secure, and no one from SimpleCare Insurance will ask you to do it.

Can I get health insurance if I am not a U.S. citizen?

Can I get Marketplace coverage in Miami if I only have an ITIN?

An ITIN alone does not establish eligibility. To enroll through the Marketplace you generally need to be lawfully present in the United States and not have access to other qualifying coverage; many lawfully present immigrants qualify for coverage and for financial help. An ITIN is a tax number, and Simple Tax can file with one. Bring your immigration document and we will check together, free.
